Add Questions to a Zoom Registration Form

You can customize the Zoom event registration form to ask for specific pieces of information, or to add custom questions.  Custom questions can either be "short answer" or multiple choice (though only one option can be selected in their response).

Once you have created your event, scroll to the bottom of the event page.  On the Registration tab, click edit to the right of the Registration Options:




The Registration window is displayed, and you can choose which fields of information are requested (and required) on the Questions tab (first name and email address are always required):



On the Custom Questions tab, you can add your own question(s) and require them before a successful registration.  Single answer is the somewhat confusingly named option for multiple choice questions; I believe they chose to call it that because you can't create a question that accepts more than one answer.



Be sure to save each page as you complete it, and the questions will appear on the registration form.  You can review registrants' responses by clicking View on the Manage Registrants section of the event page (see first image, above).
